If you do not have the Baptism Certificate
please read the following note

On certain occasions, a candidate is not able to provide a copy of the Baptism Certificate. If the candidate has exhausted all avenues and the Certificate was not found, in those cases, the candidate should present the Affidavit of Baptism.

• The affidavit must be completed in front of a priest, deacon, or catechetical leader. 
• A picture of the person being baptized must be attached to the Affidavit
• The affidavit will replace the certificate during the registration period, BUT the candidate must provide a copy of the certificate of baptism by December of this year.
  

En ciertas ocasiones, un candidato no puede proporcionar una copia del Certificado de Bautismo. Si el candidato ha agotado todas las vías y no se encontró el Certificado, en esos casos, el candidato debe presentar la Declaración Jurada del Bautismo. 

• La declaración jurada debe completarse frente a un sacerdote, diácono o líder catequético
• Una foto de la persona siendo bautizada debe adjuntarse al Afidavit
• El affidavit reemplazara el certificado durante el periodo de inscripcion, PERO el candidato debe presentar una copia del certificado de bautismo para Diciembre de este año.
