• If you pay by check or Money Order send the check Payable to Carlos Bayola to

• If you pay by Zelle you must Send the form by Text to 305-519-9041

 Directions on how to use your Coupon Code

Once you have sent this form with the Mass Selected and the Form of Payment to Carlos Bayola,

• You will receive a text message with the Photographer’s Page link along with the Coupon Code (a kind of E-Gift Card)
• Please after you receive this text immediately respond stating that you have received the Coupon Code.
• Then once the Photos are uploaded (usually 1 to 2 weeks after the ceremony) you will need to follow these steps:

1. Log into the following website;  www.carlosbayola.smugmug.com
2. Click on Our Lady of Guadalupe Catholic Church
3. Click on Communions Gallery Folder
4. Click on the Folder of your child’s Date & Time
5. Once inside the folder- Click on the green “BUY PHOTOS” button on the right-hand corner
6. Click -Photos from this gallery
7. Select your photos and click on the green button “Add photos to cart”
8. Note: To change the size of the image your purchase, click on the change product bar underneath the size of the print.
9. If you haven’t finished selecting all images, click from the option box “select more photos”
10. If you have finished selecting all images, click the green bar “CHECKOUT”
11. Once you have selected all your images for the proper amount being credited, please click on the green CHECKOUT button.
12. Your Shopping Cart will appear on the screen, right beneath Your Shopping Cart there’s a green Have a Coupon?  with a white Coupon Code: box,
13. Inside the white box, please insert your coupon code and hit apply.  Your coupon credit will be applied at checkout.
14. Click on Checkout (green button)
15. Insert your shipping information
16. Click on NEXT: Shipping Options
17. Click on NEXT:  Payment Info
18. NOTHING DUE!
19. Click on NEXT: Review Order
20. Review your order and you are done!
